Customization of intake forms

I would like to be able to have the Sadio portal send my patients intake forms that I have personally customized prior to their checkin arrival. I have my own questions that I care about rather than the generic EZDerm templated ones. For efficiency, I would like these sent to the patient before they arrive at their appointment through the portal. 

1) Personalized intake forms

2) All check-in forms/registration completed through the portal prior to arriving at appointment so that check-in process is more efficient. Patient should not have any further paperwork to complete when they arrive other than give their name to front desk. There is no reason with today's technology that any patient should have to do any paperwork when they arrive if all the forms can be sent to them beforehand through the portal.
