Printable Version of EZ Check In

For patient's who are not able or willing to use the iPad to enter their medical history, you can print a version of EZ Check In (see attachment). After the patient has filled this out via the paper form the clinical staff will need to manually enter this into the patient's history section in EZDERM. 

Please note that this is a generic version of the EZ Check In workflow. Feel free to download and edit the template to mirror the exact data you have set in the app.
