This article will outline how to create a new patient (or chart) within the EZDERM Practice Management System. There are two ways to create a new patient: either from the Schedule or via the Patients tab.
On the Schedule:
1. Double click on the calendar as if you were creating a new appointment.
2. Start typing in the name in the "Patient" field at the top.
3. If you do not see that patient's name in the system already, at the bottom of the drop-down list you will see the button to Create New Patient.
4. Once you click on "Create New Patient", you will be taken to the Create Patient screen where First/Last Name, Date of Birth, Gender, Ethnicity, and Race are mandatory fields.
Once you create a patient, you will see the details populated. But even if you decide not to create an appointment the new patient will remain in the system.
Via the Patients tab:
1. Click on "Patients" on the left-hand side.
2. Click on "Create New" in the upper right-hand corner.
3. Once you click "Create New", a Demographics window will open. In addition to the mandatory fields mentioned earlier, an address will also be required. In this screen, you will also be able to add details such as Preferred Communication, Related Person, and more. Those details can be used in cases where a patient is a minor.
