1. Click on the practice name or logo in the upper left-hand corner, select General Settings to open a drop-down menu and click on Consents.
2. Click Create New Consent in the top right.
3. Enter your consent title/name and choose a consent type.
One Time: Appears for first-time patients in EZ CHECK IN and Sadio Patient Portal.
Encounter Specific: Tied to chief complaint and appears in EZ CHECK IN and Sadio for patients who are scheduled with that CC.
Procedure Specific: Tied to a procedure and will appear as an unsigned consent once the procedure has been added to the body map.
Medication History Specific: There only needs to be one of these types of Consent in your practice settings to allow for use of the RX Eligibility, Prescription History & Formulary Module. A default out-of-the-box version will be pre-loaded into EZDERM for you.
4. Designate if a cosigner is required (Provider and/or Witness).
5. Enter the body of the consent into the Consent Text field.
A blank version of any Consent can be downloaded as a PDF by clicking the PDF icon to the right of a Consent name when looking at the list of consents.

Note: There is not an option to have a patient initial on specific lines or document free-text within the consent. They are simply asked to sign at the end of the consent. (see image below)
Once the patient signs the consent on the EZ CHECK IN kiosk or Sadio Patient Portal the consents will be automatically saved in the Consent folder under documents in the patient's chart.
