If you will be billing insurance (submitting claims) through EZDERM and our practice management system, our enrollment team is here to help you get going! They will need a few forms filled out to provide them with some basic practice information as well as the payers you will be working with. They will then work to get you enrolled with our clearinghouse, Change Healthcare, for all of those payers. Please note that, depending on the payer, enrollment may also be required to run insurance eligibility checks in the system.
These forms were sent along to the office via email but can also be found attached at the bottom of this article. The forms must be filled out electronically. Any forms filled out by hand will have to be completed electronically and resubmitted.
The forms should be completed and returned to enrollment@ezderm.com.
