To document an Excision and Repair:

1. Select your excision treatment plan and apply it to the body map. Please Note: If the Chief Complaint or one of the complaints in the HPI has a planned Excision procedure, the plan will already be listed in the Physical Exam for you to execute. Execute the plan by first tapping on the text from the top menu that states "no active treatment plan."  You will then tap on the circle to the left of the treatment plan and apply it to the location on the body map. Once applied you will see the selection wheel, and then you will tap on the blade holder icon with the + to apply the plan.

3. To apply a repair to the visit, you will want to tap the red plus sign to the right of the plan(this will link that plan under the same diagnosis).  Once selected you will then apply it by tapping on the procedure tag, and selecting the blade holder icon with the plus sign.

5. Once your procedures have been applied, you can then enter in the procedure details (i.e. dimensions, morphology, photo, etc.) You will do this by tapping once on the procedure tag on the body map.  Once the selection wheel pulls up, you will then select the blade holder icon without the plus sign.  This will pull up both procedures (excision and repair).  You will open each template by tapping into it and then continue with entering in all of your information. 

Please Note: You must enter Post-Operative Dimensions in each procedure for the system to generate the appropriate CPT code.
