EZDerm Insurance Policy Type cheat sheet

When adding insurance information to a patient's chart, you will only use the following four insurance types. This guide includes examples and images of each insurance card type.

1. Blue Cross and Blue Shield- You will use this option for ALL Blue Cross and Blue Shield plans to include: Medicare Advantage, HMO, PPO, Anthem, ect. Below are some photo examples of these insurance cards. 

2. Commercial Insurance Co.- You will use this option for ALL private insurances to include: Medicare advantage plans that are NOT Blue Cross and Blue Shield, VA, Secondary (to Medicare part B) Insurance plans, Aetna, United Healthcare, ect. Below are some photo examples of these insurance cards.

3. Medicaid- If your office accepts Medicaid you will select this option for any Medicaid Policy. Below are some photo examples of these insurance cards.

4. Medicare Part B- You will use this option IF Medicare Part B is your patient's primary insurance and they DO NOT have a Medicare Advantage Plan. Below is a  photo example of this insurance card.
