Important Note: To meet Measure #130, clinicians must document that they obtained, updated, or reviewed the patient’s current medications—"including all prescriptions, over-the- counters, herbals, vitamins, minerals, dietary (nutritional) supplements, and cannabis/cannabidiol products with each medication’s name, dosage, frequency and administered route."

That said, CMS allows for some flexibility: if the clinician makes their best effort to document a current, complete, and accurate medication list utilizing all resources at the time of the encounter they can still meet the measure, BUT the clinician must document that the medication information is unavailable/unknown.
