TIN Confirmation

As part of the practice registration process, Healthmonix is required to validate the Tax ID Number that is to be associated with your submission, should you choose to submit to CMS through the MIPS PRO tool.

In order to verify your TIN, you will need a copy of a completed HCFA-1500 claim form. Please provide this as a PNG or JPEG image and upload the form by clicking on choose a file within your MIPS Pro account.

Once this file is uploaded, Healthmonix will verify it automatically. If the verification is successful, this step will be complete. If there is an issue, Healthmonix will contact you within 2 business days.
